Please be prepared to pay the $50 application fee. The application fee is NON-REFUNDABLE, no exceptions. In addition to this Salon Suite application, you will also be required to provide a copy of a valid form of identification and TWO FULL MONTHS of proof of income. All documentation should be in PDF format. Word, JPG and PNG uploads may have to be re-submitted and we will not be held responsible should this delay your application, or cause your application to be denied.
To complete this rental application, you must provide a copy of your TDLR license. You will also be asked to provide a Mini-Salon license upon approval! If your service does not require a TDLR license, this must be disclosed to Ashby prior to submitting the application.
